As well, they can be used in a kink setting too. I’m a trans woman that worked with their doctor in order to induce lactation. I needed to use a breast pump, alongside medication, to do that. It makes me really euphoric, and even though it’s only a small amount, I like being able to breastfeed sexual partners because it feels really nurturing.
You do need to maintain it. I have chronic health issues that cause nausea, and Domperidone (the med I take for lactation) is a good anti-emetic as well. Then, milking with a pump is required to maintain a high amount of production, but there’s always a little bit for me.
